Case history
A 28-year-old woman presents with a history of severe pain on defecation for the last 3 months. She has noticed a small amount of blood on the stool. The pain is severe and she is worried about the pain she will experience with the next bowel action.
Other presentations
Pain after defecation typically lasts for up to an hour, but may be of longer duration and sometimes lasts for many hours. The presence of a skin tag in midline posteriorly in association with painful defecation and anal spasm is almost diagnostic of an anal fissure.
