A 22-year-old college student presents with 3 days of right eye pain, redness, photophobia, and a progressive decrease in his vision. He complains of yellowish discharge and mild right lid swelling. The patient is a contact lens wearer and upon further questioning admits to falling asleep and wearing his contact lenses overnight 4 days ago while studying for a final exam. Corneal scraping by an ophthalmologist, sent for Gram and Giemsa stains and culture, confirms a diagnosis of Staphylococcus aureus keratitis.
Case history #2
An 81-year-old woman is brought in by her caregiver. One week ago the caretaker noted red scaly lesions on the left side of the patient's forehead and nose. The skin lesions do not cross the midline. Two days ago the left eye became red and teary. The patient has been complaining of decreased vision and discomfort in the left eye, as well as itching and tingling of her left forehead and scalp. The patient was referred to ophthalmology with a presumed diagnosis of varicella zoster keratitis.
