Case history #1
A 58-year-old white woman has clustered microcalcifications in the right breast on routine mammography screening, which were not seen on her previous mammogram. She is postmenopausal, has used hormone replacement therapy for 6 years, and has a BMI of 26. She has one sister who was diagnosed with breast cancer.
Case history #2
A 55-year-old postmenopausal black woman presents with 2 new areas of breast nodularity that do not resolve. Mammography is negative.
Other presentations
Sixty percent of pure ductal carcinoma in situ is detected mammographically, most commonly in association with clustered microcalcifications. Other cases are found in women presenting with symptoms, such as nipple discharge (which may be bloody) or Paget disease. There is no classic mammographic pattern for lobular carcinoma in situ. The disease is multifocal or multicentric in 40% to 90% of cases, and similar findings are present in the opposite breast in up to 50% of cases.
