Atypical antipsychotics
Atypical antipsychotics (e.g., olanzapine, quetiapine, aripiprazole) require further study. Small randomized controlled trials suggest a possible role in social anxiety disorder.[104]Williams T, Hattingh CJ, Kariuki CM, et al. Pharmacotherapy for social anxiety disorder (SAnD). Adverse effects of weight gain and metabolic syndrome may limit usefulness in some patients.
Buspirone
While some authors support the use of buspirone for augmentation of other treatment strategies, efficacy as a monotherapy for social anxiety disorder has not been demonstrated in controlled trials.[108]Kimmel RJ, Roy-Byrne PP, Cowley DS. Pharmacological treatments for panic disorder, generalized anxiety disorder, specific phobia, and social anxiety disorder. In: Nathan PE, Gorman JM, eds. A guide to treatments that work. 4th ed. Cycloserine augmentation
Increasing attention is being invested toward better understanding how to improve exposure-based outcomes for anxiety. Cycloserine is a partial N-methyl-D-asperate agonist. Low doses can be used to enhance the efficacy of exposure-based treatments for social anxiety through memory consolidation.[149]Guastella AJ, Richardson R, Lovibond PF, et al. A randomized controlled trial of D-cycloserine enhancement of exposure therapy for social anxiety disorder. Biol Psychiatry. 2008 Mar 15;63(6):544-9.

An individual participant data meta-analysis on cycloserine augmentation for exposure-based cognitive behavioral therapy found that patients randomized to cycloserine augmentation had a small but significant reduction in symptom severity at post-treatment and follow-up compared with those who received placebo.[151]Mataix-Cols D, Fernández de la Cruz L, Monzani B, et al. D-Cycloserine augmentation of exposure-based cognitive behavior therapy for anxiety, obsessive-compulsive, and posttraumatic stress disorders: a systematic review and meta-analysis of individual participant data. These findings were not moderated by concurrent antidepressant use or the number of therapy sessions.[151]Mataix-Cols D, Fernández de la Cruz L, Monzani B, et al. D-Cycloserine augmentation of exposure-based cognitive behavior therapy for anxiety, obsessive-compulsive, and posttraumatic stress disorders: a systematic review and meta-analysis of individual participant data. http://www.ncbi.nlm.nih.gov/pubmed/28122091?tool=bestpractice.com
In one subsequent randomized controlled trial, cycloserine augmentation was equally effective when administered before or after exposure treatment.[152]Smits JAJ, Pollack MH, Rosenfield D, et al. Dose timing of D-cycloserine to augment exposure therapy for social anxiety disorder: a randomized clinical trial. JAMA Netw Open. 2020 Jun 1;3(6):e206777.

However, some have questioned the clinical significance of the small effect and highlight the mixed results of individual studies.[65]Bandelow B, Allgulander C, Baldwin DS, et al. Further research is focused on personalization in an attempt to maximize the efficacy of augmentation strategies.
Interpersonal psychotherapy
This effective treatment for depression has also yielded promise as a psychological intervention for social anxiety disorder, but it may be less effective than cognitive behavioral therapy for social anxiety disorder.[77]Mayo-Wilson E, Dias S, Mavranezouli I, et al. Interpretation-modification therapy
Attentional bias towards threat is considered to be associated with the maintenance of social anxiety. Training individuals with high levels of social anxiety to interpret ambiguous social cues as benign has been found to reduce attentional biases to social threat cues.[157]Amir N, Bomyea J, Beard C. The effect of single-session interpretation modification on attention bias in socially anxious individuals. J Anxiety Disord. 2010 Mar;24(2):178-82.

Transdiagnostic approaches
Transdiagnostic therapy involves taking a cognitive-behavioral approach to managing anxiety, regardless of specific diagnoses. The unified protocol is an example of this approach, with treatment focusing on targeting shared characteristics across disorders, such as negative affect and emotion dysregulation. Research supports the efficacy of transdiagnostic therapy for social anxiety, including comparable findings to disorder-specific protocols and presentations with comorbid depression.[88]Dear BF, Staples LG, Terides MD, et al. Exercise augmentation
Symptomatic outcomes for cognitive behavioral therapy in social anxiety disorder can be improved with the augmentation of aerobic and anaerobic exercise.[162]Jayakody K, Gunadasa S, Hosker C. Exercise for anxiety disorders: systematic review. Br J Sports Med. 2014 Feb;48(3):187-96.

Alpha-Stim AID cranial electrotherapy stimulation
An electrotherapy device that aims to reduce symptoms of anxiety by generating alpha brain waves. One small open-label study with no control group showed improvement in Patient Health Questionnaire (PHQ-9) and Generalized Anxiety Disorder (GAD-7) scores among patients with anxiety symptoms after 6 weeks of treatment.[164]Griffiths C, da Silva K, Jiang H, et al. Alpha-Stim AID cranial electrotherapy stimulation (CES) anxiety treatment: anxiety, depression and health-related quality-of-life outcomes in primary health-care social prescribing services. However, the mechanism of action is unclear and there are no studies comparing Alpha-Stim AID with other evidence-based standards of care for anxiety.[165]National Institute for Health and Care Excellence. Alpha-stim AID for anxiety disorders. Mar 2021 [internet publication].
