Complications
Patients with underlying chronic lung disease or emphysema may develop chronic cavitary histoplasmosis, which presents with productive cough, weight loss, fevers, malaise, and possibly haemoptysis.
Without treatment the disease is progressive and may result in death.
This is an uncommon but documented manifestation of disseminated histoplasmosis.
Following intensive exposure, patients with acute pulmonary histoplasmosis may show diffuse reticulonodular infiltrates on chest x-ray and have respiratory distress, and the condition may progress to respiratory failure. See Acute respiratory failure

Treatment is more likely to fail in the context of an impaired host or certain forms of disease (chronic pulmonary disease). It may manifest as a failure to respond to therapy or as disease recurrence.
