Probiotics have been suggested as a modality for preventing AOM. However, a meta-analysis reported that probiotics do not reduce the incidence of AOM in children who are prone to otitis media.[51] Probiotics may reduce the incidence of AOM in children who are not prone to otitis media, but the authors advise caution because of inconsistencies in sub-group analysis between trials.[51] Evidence for the impact of probiotics on the severity and duration of AOM, need for antibiotics, antimicrobial resistance, hearing loss and need for specialist referral is lacking. The optimal probiotic regimen is unclear; further randomised controlled trials are needed.

Nasal spray
An alpha-haemolytic streptococcal nasal spray may reduce the number of recurrences of AOM in children prone to ear infections.[52]
