Case history

A 21-year-old Hispanic woman presents with new, asymptomatic white spots that were initially noted 12 months ago on her dorsal hands. Over the following months, she has noted new similar spots symmetrically on her eyelids, around her mouth, and on her upper and lower extremities. Some of the affected areas have gradually enlarged. She denies any associated redness, itching, or scaling prior to the appearance of these white spots. On physical exam there are well-demarcated and depigmented patches of skin, without any texture changes, in a symmetric distribution over the head and neck, torso, genitals, and extremities.

Other presentations

Other rare presentations of vitiligo include inflammatory vitiligo (inflammatory infiltrate at the tissue level reflected in a slightly raised erythematous border, often with scale, of actively enlarging lesions), trichrome vitiligo (a third color, a shade between depigmented and normally pigmented skin, is present), and confetti-like depigmentation (many tiny 1-2 mm macules of depigmentation in clusters). These presentations frequently mark highly active disease.[3][4][5]
