Formal risk assessment tools when used alone do not accurately predict future self-harm for a specific individual, and clinicians can be falsely reassured or misled. Many risk factors are so generic that they are of little help in the clinical determination of suicide risk for any individual patient.[131] A compassionate and collaborative discussion can produce a tailored safety plan that addresses the patient’s current concerns and includes ways they can deal with any future crises. A safety plan belongs to the patient and ideally can be shared with the clinician and reviewed regularly as part of routine discussions, and at change/transition points.
