POF can occasionally be characterized by intermittent spontaneous restoration of ovarian function, particularly in the early stages of the disease, but in general, the disease course is not reversible. Women diagnosed with this condition are at increased risk of osteoporosis, cognitive impairment, and cardiovascular comorbidity.[33][38]​​ If treated appropriately with hormone replacement therapy until age 50, and with careful screening and management for associated autoimmune disorders, women with a diagnosis of POF can expect healthy long-term outcomes. Additionally, successful pregnancies can be achieved with utilization of donor oocytes and assisted reproductive technologies.
