Develops acutely in untreated severe serotonin toxicity due to prolonged tonic-clonic muscle activity in association with hyperthermia. Characterised by a rising creatine phosphokinase level. Can be prevented with early treatment of severe serotonin toxicity with muscle paralysis and cooling.[24][25][26]

Develops in the same setting as rhabdomyolysis and is due to untreated severe serotonin toxicity. Patients develop multi-organ failure with disseminated intravascular coagulation, acute renal failure, and gradual involvement of other organs. Can be prevented with early treatment of severe serotonin toxicity.[8][24][26]
