Most patients with acute interstitial nephritis (AIN) are left with some degree of renal impairment.

In one study of 205 patients with AIN, 50 patients (27%) with no recurrences and 12 patients (55%) with recurrent AIN developed stage 4 or 5 chronic kidney disease over a median follow-up of 30 months.[36]

All patients require annual monitoring. If disease progression is detected, treatment with ACE inhibitors may be required. Medications causing AIN should be avoided in the future.

Some patients may develop hypertension as a consequence of chronic kidney disease.

All patients require annual BP monitoring.

Patients with chronic kidney disease require ACE inhibitor therapy, but additional antihypertensive medication may be required if hypertension is not adequately controlled by ACE inhibitors alone.
