A 32-year-old woman has just been confirmed by ultrasound scan as pregnant with twins at a gestational age of 10 weeks. She had been trying for a pregnancy for the past 5 years, but has had no sub-fertility treatment. On her way home, she notices bright red vaginal bleeding. She is not in any pain. She has no postural dizziness.

A family attends a 12-week dating scan. The midwife sonographer appears concerned during the scan and obtains a second opinion. The second sonographer who undertakes the ultrasound examination confirms the absence of visible heart activity of the embryo.
Other presentations
A few women may complain of a lower abdominal or lower back pain in association with vaginal bleeding, but in most patients it is a painless event. Some patients may attribute an event of temporal relationship to the onset of symptoms (e.g., a fall, a direct blunt abdomino-pelvic trauma, a cold, or intake of some medication). It may very occasionally follow a speculum examination. A few women do recall some brownish or dirty vaginal staining in the preceding days or weeks. Pregnancy may have been confirmed a few days or weeks earlier, or may not have been suspected at all.
