Complications
Systemic absorption of topical medications can occur when there are large areas of erosions or fissuring present on the skin. If salicylate-containing preparations are used, toxic amounts may be absorbed.[33]
Patients with X-linked ichthyosis are at a higher risk for testicular cancer and need to be followed appropriately with yearly testicular examinations by a physician and monthly self-examinations.
Patients with lamellar ichthyosis may develop alopecia.
This outward turning of the lower eyelid can sometimes develop in patients with lamellar ichthyosis. Initial treatment may involve use of artificial tears. Topical retinoids applied to the lower lid can lead to improvement. Regular examinations with an ophthalmologist are important in ensuring that there is no ocular damage.[32]
The long-term adverse effects of retinoids are well described and include elevations in triglycerides and calcification in the tendons and/or ligaments. Patients who are placed on these medications need to be followed with routine laboratory monitoring (including FBC, fasting lipids, and liver enzymes) and x-rays for symptomatic joints.[25]
The thicker, corrugated scales associated with epidermolytic ichthyosis can lead to increased bacterial colonisation of the skin and development of a foul odour. Although this is often not threatening to the overall health of the patient, it can be socially disabling. The use of dilute bleach baths, antibacterial soaps, and topical mupirocin creams can be helpful in treating this if it develops.[32]
