Develops acutely in untreated severe serotonin toxicity due to prolonged tonic-clonic muscle activity in association with hyperthermia. Characterized by a rising creatine phosphokinase level. Can be prevented with early treatment of severe serotonin toxicity with muscle paralysis and cooling.[24][25][26]

Develops in the same setting as rhabdomyolysis and is due to untreated severe serotonin toxicity. Patients develop multiorgan failure with DIC, acute renal failure, and gradual involvement of other organs. Can be prevented with early treatment of severe serotonin toxicity.[8][24][26]
