A 28-year-old woman presents for prenatal care at 8 weeks' gestation. Routine screening with the fourth-generation antigen/antibody combination enzyme-linked immunosorbent assay is reactive, and confirmatory testing with an antibody differentiation immunoassay is positive for HIV-1 antibody. Laboratory results are consistent with HIV-1 infection.
