A 55-year-old man with a history of chronic pain and depression presents to the emergency department with rapid breathing and agitation. He is found to have tachycardia and dilated pupils. Further questioning and review of medical records reveal longstanding use of both prescription and street drugs.
Case history #2
An 18-year-old pregnant woman presents with altered sensorium and difficulty breathing. She reports feeling “overwhelmed” and difficulty sleeping. There is a history of suicidal attempts in the past that have resulted in hospitalization. Physical exam reveals inflamed nasal mucosa and constricted pupils, along with multiple needle tracks on the nondominant arm.
