Case history
An opposite-sex couple presents to a fertility clinic as they were unable to achieve pregnancy after 1 year of unprotected intercourse. The woman is 28 years old with regular and normal menses. A hysterosalpingogram shows normal uterine contour and patent tubes bilaterally. The male partner is 29 years old with a negative medical history. Examination is unremarkable, with normal testicular volume and secondary sexual characteristics. Two semen analyses separated by 1 month show low sperm concentration (10 million/mL), decreased sperm motility (20%), and increased percentages of sperm with abnormal morphology.
Other presentations
The patient may present with an existing medical condition that is known to affect male fertility, including varicocele, cystic fibrosis, prior chemotherapy or radiotherapy, hypospadias, cryptorchidism, and Klinefelter syndrome. Patients may also present with erectile dysfunction and signs of endocrinopathy.
