Case history #1

A middle-aged woman reports difficulty falling asleep. She has been treated for anxiety in the past and is perimenopausal. She describes drinking her last cup of coffee at 9 p.m. before retiring at 10 p.m., then watching the time on her alarm clock on and off for an hour before falling asleep. Her physical examination and laboratory tests are normal.

Case history #2

An older man presents to his geriatrician with symptoms of waking several times in the night and also rising too early in the morning. He reports multiple medical problems, including hypertension, enlarged prostate, and mild heart failure. He denies drinking alcohol or caffeine prior to bedtime. On physical examination, he is very thin with 1+ pedal oedema.
