This self-assessment and planning guide is intended to help schools identify the strengths and weaknesses of their health promotion policies and programs, develop an action plan for improving student health, and involve teachers, parents, students, and the community in improving school services. It focuses on physical activity and nutrition. After presenting a physical activity and nutrition fact sheet and instructions for the site coordinator, the guide offers eight modules that look at: school policies and environment; health education; physical education; nutrition services; school health services; school counseling, psychological, and social services; health promotion for staff; and family and community involvement. The modules include instructions for coordinators, scorecards, questionnaires, and planning questions. Resources for each module are attached. (SM)
