School Health Index for Physical Activity and Healthy Eating: A Self-Assessment and Planning Guide. Middle School/High School.

This self-assessment and planning guide is designed to help schools evaluate their physical activity and nutrition policies and programs based on national standards and guidelines. It assists schools in identifying strengths and weaknesses of their health promotion policies and practices, developing action plans for improving student health, and involving teachers, parents, students, and the community in improving school services. The first section offers instructions for the site coordinator, which include: review the modules, assemble the school health index team, meet to discuss the index and assign tasks, complete each module, complete the overall score card, and meet to develop an action plan. The guide's eight modules present information on: (1) "School Policies and Environment"; (2) "Health Education"; (3) "Physical Education"; (4) "Nutrition Services"; (5) "School Health Services"; (6) "School Counseling, Psychological, and Social Services"; (7) "Health Promotion for Staff"; and (8) "Family and Community Involvement." Each module presents instructions for the coordinator, a questionnaire, planning questions, and worksheets. A list of resources is included. (SM)
