The School Information Form was developed in conjunction with the Pennsylvania Plan (see TM 000 608) to provide data for assessment of educational quality in Pennsylvania public schools. The form was completed by a school representative or staff member at the sample school. Items provide such information as the size of school population, the number and type of school personnel, housing characteristics of the community, library facilities, and holding power of the school.
