The Case Management Model: Implementation Requirements. Volume II.

This guide describes the social services case management process in detail, focusing on some of the ways in which the basic process requirements can be met. An explanation of why each step in the process is necessary is offered from both a system and a client point of view. The guide is divided into seven chapters, each of which concentrates on one of the basic requirements of the case management process. The seven requirements examined are: (1) evaluation of service request or need, (2) eligibility determination, (3) planning of service objectives, (4) arrangements for linking client with service providers, (5) provision of services, (6) overseeing the case, and (7) recordkeeping. Each of these requirements is explained according to purpose, process and content with an emphasis on the activities and tasks which are thought to be most useful in maintaining a deliberate coordinating and organizing role at the individual case level. Techniques that are viewed as common to good practice are incorporated but not explored in detail. This guide is keyed to a series of videotapes intended for use in training social service workers in case management techniques. (JMB)
