Transitioning to Post-School Readiness for Students with Moderate to Severe Intellectual Disabilities

The purpose of this dissertation in practice was to increase post-school success by ensuring parental/guardian education and partnerships with post school agencies and programs. Study participants consisted of the parents or guardians of 18-22 year old students with moderate to severe disabilities enrolled in the Supportive Transition Exploration and Pre-vocational Skills (STEPS) program at Kent County Community. Participants were surveyed on knowledge of select agencies and programs. Participants were then able to access an online Transition Dashboard created for this project, that featured short videos of each program or agency and pertinent information on contact and application information. Following the survey conferences and/or a user guide manual for the Transition Dashboard was provided and participants were able to review the videos and materials featured on the Transition Dashboard. After review of materials, participants completed a post-survey. Findings support the need for specific educational materials on postsecondary options for students with moderate to severe disabilities. The project also found that educational materials such as the Transition Dashboard increase participants' understanding of postsecondary options and potential providers to be a part of their partnership network team.
