The 14 appendixes comprising the document are: a list of Talent Search advisory council members, analysis advisory committee members, and other consultants; OE Form 1231, in which Talent Search project directors indicate client population, project accomplishments, and financial assistance information; DHEW regional boundaries and headquarters; materials related to sampling methodology and sampling error computation and data collection and processing procedures; Talent Search site-visit protocol; the Talent Search project director questionnaire; the Talent Search counselor questionnaire; the Talent Search client educational form; materials concerning participant eligibility and program legislation for TRIO programs; a form sent to project directors for listing clients who began studies at a postsecondary institution between July 1 and December 31, 1973; a form sent to registrars of postsecondary institutions to verify enrollment status of former Talent Search clients; and OE Form 1227, a quarterly financial report for special programs.
