A Standardized Certification Program for Case Managers Serving Frail Elderly Texans. Module II: Assessment and Care Plan Development.

This learning module is one of three training modules that were developed for members of the Texas Gerontological Consortium for Continuing Education to use in preparing case managers working in human service professions coordinating community-based programs for frail elderly Texans. Module II deals with the following topics: assessment (role of assessment; purpose and value of elderly assessment; criteria for instrument selection; basic concepts of aging and personality, aging and intelligence, sociometric assessment, and observations/interviews; functional assessment; assessment in the public sector by case managers); age-related mental and physical changes (psychosocial aspects of disabilities, consequences of bed rest, changes in the various body systems, sensory changes, kinesthetic and vestibular senses, mental health conditions); writing a care plan; and practicum. Included in the module are the following components: estimate of time required to complete the module; a list of suggested videos; and topic outlines containing topic objectives, the information to be learned for mastery of each topic objective, 49 references, and transparency masters.
