A previously healthy 10-month-old boy presented to the emergency department with a 1-week history of progressive swelling of the right upper eyelid (figure 1). He had a recent history of respiratory tract infection but no history of fever, insect bites or trauma. He was alert in good general condition, and the urine output was maintained. No weight loss was observed. Physical examination revealed erythema, oedema and ptosis of the right upper eyelid without warmth or tenderness. No other rashes were present on the skin including the scalp. Ocular motility disorders, displacement and protrusion were not observed. Otitis media and otitis externa were also not detected. There was no evidence of hepatosplenomegaly or generalised lymph node enlargement. The haematological and biochemical test results were normal. Contrast-enhanced CT of the head revealed bone defect and soft tissue swelling of the right superior orbital rim (figure 2A).
